New Patient Forms, Informed Consent and Questionnaires
We have put together our forms in one place so that you can fill them out at your leisure. Once filled out, you can mail them to our office or bring them with you to your first appointment. It is understood that by filling out these forms you agree to comply to Natural Health Improvement Center's Terms and Conditions as stated in the form. If a further signature is needed, we will have it ready for you to sign when you come in for your appointment.
